Zinc Deficiency: Signs, Causes, Testing, and How to Correct It Safely

Zinc may be a trace mineral, but its job description is anything but small. It is involved in hundreds of enzymatic reactions and helps regulate immune function, wound healing, DNA and protein synthesis, reproduction, antioxidant defenses, and our ability to taste and smell.

Because zinc supports so many systems, inadequate zinc status can create a surprisingly broad collection of symptoms. The challenge is that most of those symptoms are not unique to zinc deficiency. Fatigue, hair thinning, frequent infections, skin changes, or altered taste can also be associated with thyroid dysfunction, iron or vitamin B12 deficiency, inadequate protein intake, chronic inflammation, gastrointestinal disease, medication effects, and other conditions.

In other words, zinc deserves attention—but it should not be asked to take the blame for every brittle nail or bad hair day.

What Does Zinc Do?

Zinc is essential, meaning the body cannot manufacture it and must obtain it through food or supplementation. It contributes to:

  • Normal immune-cell development and function
  • Skin integrity and wound healing
  • DNA synthesis, cell division, and tissue repair
  • Protein synthesis and growth
  • Normal taste and smell
  • Reproductive and hormone function
  • Antioxidant and inflammatory regulation

The body does not maintain a large, readily available storage depot for zinc. Consistent dietary intake and healthy absorption are therefore important.

Possible Signs of Zinc Deficiency

Zinc deficiency can range from mild or marginal insufficiency to severe deficiency. Possible findings include:

  • Reduced or altered taste and smell
  • Poor appetite
  • Frequent or prolonged infections
  • Slow wound healing
  • Dermatitis or unexplained inflammatory skin changes
  • Hair thinning or hair loss
  • Diarrhea
  • Impaired growth or delayed sexual maturation in children and adolescents
  • Reproductive problems in adults
  • Fatigue or difficulty concentrating

Some people also report brittle nails or white spots on the nails. These findings are not specific to zinc deficiency, however. White nail spots—known as leukonychia—are commonly caused by minor trauma to the nail matrix and should not be treated as a zinc test conveniently attached to the end of a finger.

Who Is Most at Risk?

1. People eating predominantly plant-based diets

A well-planned vegetarian or vegan diet can be very nutritious, but zinc deserves deliberate attention. Animal foods generally contain more readily absorbed zinc. Legumes, soy foods, nuts, seeds, and whole grains contain zinc, but they also contain phytate, a natural plant compound that binds zinc and reduces its absorption.

This does not make beans, tofu, seeds, or whole grains “bad” foods. It means that total zinc intake on a plant-based diet may look adequate on paper while the amount absorbed is lower.

Traditional preparation methods can help. Soaking, sprouting, fermenting, and leavening foods can reduce phytate and improve mineral bioavailability. Examples include tempeh, fermented sourdough bread, soaked or sprouted legumes, and sprouted seeds or grains.

2. People with chronic diarrhea or gastrointestinal disease

Zinc is absorbed primarily in the small intestine. Chronic diarrhea can both reduce absorption and increase zinc losses. Risk may also rise with inflammatory bowel disease, celiac disease, short-bowel syndrome, bariatric surgery, pancreatic disease, or other causes of malabsorption.

Several loose bowel movements every day should not automatically be celebrated as “excellent regularity.” Persistent loose stools may signal that food is moving too quickly, the intestinal lining is inflamed, or nutrients are not being absorbed adequately.

3. Pregnant or breastfeeding women

Zinc requirements increase during pregnancy and lactation to support fetal growth, milk production, and maternal health.

4. People with alcohol use disorder

Alcohol can reduce zinc intake and absorption while increasing urinary zinc losses. It can also contribute to a generally nutrient-poor diet.

5. Older adults or people with limited diets

Lower food intake, reduced dietary variety, impaired absorption, chronic disease, and medication use can all increase the risk of inadequacy.

6. People taking certain medications

Some medications can affect zinc status, while zinc supplements can interfere with certain drugs. Thiazide diuretics may increase urinary zinc loss. Zinc can also reduce absorption of quinolone and tetracycline antibiotics and penicillamine if taken too close together.

How Is Zinc Deficiency Tested?

Serum or plasma zinc is the most commonly used laboratory measurement, but it is not a perfect stand-alone test. Zinc in the blood represents only a small fraction of total body zinc and can be affected by:

  • Time of day
  • Whether the test was fasting
  • Recent meals
  • Infection, inflammation, or physical stress
  • Pregnancy
  • Low albumin or other changes in protein status
  • Collection and laboratory methods

For that reason, zinc status is best interpreted using the whole picture: symptoms, diet, gastrointestinal function, medications, medical history, inflammation markers, and other nutrient levels.

Many laboratories use a serum or plasma reference interval somewhere around 60–120 mcg/dL, although the exact range varies. We usually aim for around 80–120 mcg/dL in our patients. Your provider can give you a personalized recommendation based on your situation.

When zinc deficiency is suspected, it may also be appropriate to evaluate:

  • Complete blood count
  • Iron and ferritin
  • Copper and ceruloplasmin, particularly with prolonged zinc use
  • Vitamin B12 and folate
  • Thyroid markers
  • Albumin and total protein
  • Vitamin D
  • Celiac screening or other gastrointestinal testing when indicated
  • Inflammatory markers, which can complicate zinc interpretation

How Much Zinc Do We Need?

The U.S. Recommended Dietary Allowance for adults is:

  • 8 mg per day for women
  • 11 mg per day for men
  • 11 mg per day during pregnancy
  • 12 mg per day during lactation

These amounts are designed to meet the needs of most healthy people. Someone with impaired absorption, ongoing gastrointestinal losses, or a high-phytate diet may require a more individualized strategy.

The Best Food Sources of Zinc

Food is generally the best foundation because it supplies zinc alongside protein and other complementary nutrients.

Zinc-rich animal foods

  • Oysters and other shellfish
  • Grass-fed and grass-finished beef
  • Lamb
  • Crab
  • Dark-meat poultry
  • Sardines and other seafood
  • Eggs
  • Yogurt and kefir
  • Cheese

Oysters are the undisputed zinc heavyweight. Beef and shellfish also provide zinc in a highly bioavailable form.

Zinc-rich plant foods

  • Pumpkin seeds
  • Hemp seeds
  • Cashews
  • Chickpeas and lentils
  • Beans
  • Tofu and tempeh
  • Oats and other whole grains
  • Mushrooms
  • Cocoa

Plant foods can make a meaningful contribution, especially when several sources are eaten throughout the day. Because phytate reduces absorption, vegetarian and vegan diets benefit from variety, adequate protein, fermented or sprouted foods, and thoughtful preparation.

Improving Zinc Absorption From Food

If most dietary zinc comes from plants, the following strategies may improve bioavailability:

  1. Soak and thoroughly cook legumes. Soaking and discarding the soaking water before cooking may reduce some antinutrients.
  2. Use sprouted foods. Sprouting activates enzymes that can break down phytate.
  3. Choose fermented foods. Tempeh and traditionally fermented sourdough are examples.
  4. Vary protein sources. Avoid relying on one food—such as tofu or beans—for nearly every meal.
  5. Address chronic loose stools. Correcting intake without addressing malabsorption can be a little like filling a bathtub with the drain open.
  6. Review the entire diet. Zinc deficiency may coexist with inadequate protein, iron, vitamin B12, iodine, selenium, or omega-3 intake, particularly after a major dietary transition.

Oxalates are sometimes blamed for zinc deficiency, but phytate is the more established dietary inhibitor of zinc absorption. High-calcium intake may affect zinc absorption under some conditions, but calcium-rich whole foods do not generally need to be avoided as part of a balanced diet.

When Does a Zinc Supplement Make Sense?

A supplement may be appropriate when laboratory findings, dietary history, symptoms, or a known malabsorption condition support a need. Common forms include zinc picolinate, citrate, gluconate, and acetate. Absorption differs somewhat among products, but dose, tolerability, and duration usually matter more than hunting for a supposedly magical form.

The label should be read for the amount of elemental zinc, not merely the total weight of the zinc compound.

For general maintenance, many multivitamins provide approximately 8–15 mg of zinc. Confirmed deficiency is sometimes treated with higher doses for a limited period under clinical supervision. The correct dose depends on the severity and cause of the deficiency, dietary intake, gastrointestinal function, age, pregnancy status, and other medical factors.

More Is Not Better: Zinc and Copper Balance

The adult tolerable upper intake level for zinc is 40 mg per day from all sources, unless a clinician is using a higher dose therapeutically and monitoring it. Regularly taking high-dose zinc can cause:

  • Nausea, vomiting, or abdominal discomfort
  • Reduced copper absorption
  • Copper deficiency and anemia
  • Neurologic symptoms
  • Changes in immune function
  • Lower HDL cholesterol

Long-term zinc supplementation—especially at doses approaching or exceeding 30–40 mg daily—should prompt consideration of copper status and the ongoing need for treatment. Adding copper automatically is not appropriate for everyone; it should be individualized.

Avoid intranasal zinc products because they have been associated with permanent loss of smell.

A Practical Plan for Correcting Low Zinc

Step 1: Confirm that zinc is a plausible contributor

Review symptoms, dietary patterns, bowel habits, medications, recent infections, and risk factors. Consider serum or plasma zinc along with other relevant testing.

Step 2: Identify why zinc is low

Low intake is only one possibility. Look for a recent restrictive diet, heavy reliance on high-phytate foods, chronic diarrhea, inflammatory or malabsorptive disease, alcohol exposure, or medication effects.

Step 3: Build zinc-rich foods into the diet

Include a quality zinc source daily. Omnivores might rotate grass-fed beef, lamb, shellfish, eggs, yogurt, and wild-caught seafood. Those eating a plant-based diet can emphasize pumpkin or hemp seeds, cashews, lentils, chickpeas, tofu, and tempeh while using soaking, sprouting, and fermentation.

Step 4: Supplement when appropriate

Use a dose matched to the clinical need, and avoid indefinite high-dose supplementation without monitoring.

Step 5: Reassess

Track symptoms and repeat relevant labs after an appropriate treatment interval. If zinc remains low or symptoms persist, look for continued losses, poor absorption, inflammation, another nutrient deficiency, or an entirely different diagnosis.

The Bottom Line

Zinc deficiency can affect immunity, skin health, wound healing, hair, appetite, and the senses of taste and smell. People following vegetarian or vegan diets, those with chronic loose stools or gastrointestinal disease, pregnant or breastfeeding women, older adults, and people with limited diets may need to pay particular attention to zinc.

The goal is not simply to take more zinc. It is to determine whether zinc is truly low, understand why, correct the underlying driver, rebuild the diet, and use supplementation safely when needed. That approach takes a little more detective work than grabbing the largest bottle on the supplement shelf—but it is far more likely to produce lasting results.
